📄 BLOODLINE DELIVERANCE INTENSIVE AGREEMENT

Bloodline Deliverance Intensive – Client Agreement

By signing below, you acknowledge that you have read, understood, and agree to the following terms and conditions:

1. NATURE OF SERVICE

The Bloodline Deliverance Intensive is a faith-based, spiritual ministry session focused on prayer, deliverance, and biblical guidance.

This service is not:

  • Professional counseling

  • Psychological therapy

  • Medical treatment

  • Licensed mental health care

No medical, psychological, or clinical services are being provided.

2. NO GUARANTEES OR PROMISED OUTCOMES

You understand and agree that:

  • No specific results or outcomes are promised or guaranteed

  • Each individual responds differently to ministry

  • Freedom, breakthrough, or change cannot be guaranteed

You voluntarily assume all risks associated with participation.

3. PERSONAL RESPONSIBILITY

You acknowledge that:

  • You are choosing to participate willingly

  • You are responsible for your own physical, emotional, and spiritual well-being

  • You will seek licensed professional care when appropriate

4. PAYMENT POLICY

  • Full payment is required prior to booking

  • All payments are final and non-refundable

  • This includes missed sessions and no-shows

5. RESCHEDULING POLICY

  • You may reschedule your session one (1) time only

  • Rescheduling must occur at least 96 hours (4 days) before your scheduled session

Failure to reschedule within this window results in:

👉 Forfeiture of your session

👉 No refund or reschedule granted

6. NO-SHOW POLICY

Failure to attend your scheduled session for any reason will result in:

  • Loss of session

  • No refund

  • No rescheduling

7. CHARGEBACK & PAYMENT DISPUTE AGREEMENT

By signing this agreement, you expressly agree:

  • You will not initiate any chargeback, dispute, or reversal of payment

  • You waive your right to dispute this charge through your bank or card provider

Any disputes must be handled directly with the organization.

You acknowledge that violation of this clause may result in:

  • Collection actions

  • Legal enforcement of this agreement

8. CONFIDENTIALITY & MANDATORY REPORTING

All information shared will be treated with respect and discretion.

However, you acknowledge:

  • This is not a legally privileged relationship

  • We are not bound by therapist-client confidentiality laws

We reserve the right and may be legally obligated to report disclosures involving:

  • Murder or intent to harm others

  • Child abuse or neglect

  • Elder abuse or neglect

  • Threats of serious harm or violence

By proceeding, you accept these limitations.

9. RELEASE OF LIABILITY

You agree to release, waive, and hold harmless the facilitator, organization, and all affiliated parties from any and all:

  • Claims

  • Damages

  • Injuries (emotional, spiritual, or otherwise)

  • Losses arising from participation

This includes any outcomes before, during, or after the session.

10. SESSION EXPECTATIONS

You agree to:

  • Attend the full 3-hour session

  • Be in a quiet, distraction-free environment

  • Participate honestly and respectfully

11. TECHNICAL RESPONSIBILITY

You are responsible for:

  • Internet connection

  • Zoom access

  • Being on time and prepared

Technical issues on your end do not qualify for refunds or rescheduling.

12. GOVERNING LAW & JURISDICTION

This Agreement shall be governed by and interpreted in accordance with the laws of:

👉 The State of Florida, where the organization is principally operated

👉 And where applicable, the State of Tennessee

You agree that any legal matters not subject to arbitration shall be resolved in a court of competent jurisdiction in the State of Florida.

13. BINDING ARBITRATION AGREEMENT

You agree that any dispute, claim, or controversy arising out of or relating to this agreement or the services provided shall be resolved exclusively through binding arbitration, rather than in court.

  • Arbitration shall be conducted in accordance with the rules of the American Arbitration Association (AAA)

  • The arbitration shall take place in the State of Florida

  • Each party shall bear their own legal fees unless otherwise required by law

By agreeing to arbitration, you waive your right to:

  • File a lawsuit in court

  • Participate in a class action lawsuit

  • Have disputes decided by a judge or jury

14. ENTIRE AGREEMENT

This agreement constitutes the entire understanding between the parties and supersedes all prior discussions or representations.

15. AGREEMENT CONFIRMATION

By signing below, you confirm that:

  • You have read and understood this agreement in full

  • You agree to all terms stated above

  • You are voluntarily participating
